Apple’s iOS 18.4.1 update’s release notes say it “addresses a rare issue that prevents wireless CarPlay connection in certain vehicles.”

The new software can be downloaded over-the-air on compatible iPhones and iPads, by going to “Settings” -> “General” -> “Software Update.”
The updates should be installed as soon as possible, as they also include fixes for two major vulnerabilities.
Apple says that these vulnerabilities may have been actively exploited in the wild, and that the security flaws were potentially used in an “extremely sophisticated attack against specific targeted individuals.”
